For new Sole Proprietors and Group Practices
Thank you for your interest in joining the Community Health Plan of Washington (CHPW) provider network. To us, it’s more than business — it’s a partnership that has real impact on the quality of people’s lives.
To join our network, please fill out and submit an online Provider Enrollment Form to start a New Contract Request.
We will review your information and contact you between 45 to 60 days regarding your request. Please note that some providers and practices could take up to 90 days.
