    835s for the week of December 16, 2013, will be delayed up to 48 hours. Providers should receive their 835s no later than Friday, December 20, 2013.
  • CMS 1500 Form Changes
    Starting January 6, 2014, Community Health Plan of Washington will be accepting the newly revised 1500 Claim Form (version 02/12). We will continue to accept the old 1500 Claim Form (version 08/05) through March 31, 2014 but will only accept the new Claim Form (version 02/12) starting on April 1, 2014.
