Apply for Apple Health if …
| ✔ | You are age 19 through 64 |
| ✔ | Your annual household income is at or below the Apple Health standard* |
| ✔ | You're a U.S. citizen or meet Medicaid immigration |
Apply for Medicare if …
One or more of the following is true:
| IF | You are over 65, or about to turn 65 |
| OR | You are under 65 but have certain disabilities |
| OR | You have kidney failure |
Apply for both if …
| ✔ | You are over 65, or about to turn 65 |
| ✔ | Your income is at or below the Apple Health standard |
| ✔ | CHPW's Medicare-Medicaid plan is called Medicare Advantage Dual (D-SNP) |
Apply for Individual & Family Plan if …
| ✔ | You are age 19 through 64 |
| ✔ | You want a less expensive health plan, but your annual household income is above the Apple Health standard |
*Apple Health income limit is $1,677/month for a single-person household in 2023. This amount can change every year.

Apple Health (Medicaid)Medicaid is a federal program that provides health care for people whose income is below a certain amount. Medicaid is called Apple Health in Washington State. |

MedicareMedicare is a federal program. Medicare provides health care to people 65 and over, as well as to people under 65 with specific disabilities. |

Individual & FamilyIndividual & Family Cascade Select Plans are high quality health plans offered through the State of Washington Health Benefit Exchange and can include financial support for eligible people. |
