On November 3, 2016, Community Health Plan of Washington received a notice from Summit Reinsurance Services, Inc. that CHPW member data may have been affected during a data security incident they began investigating on August 8, 2016.

We then notified the Washington State  Health Care Authority and the Washington State Office of the Insurance Commissioner, and began working with Summit to confirm whether CHPW  member data was put at risk.

On December 20, 2016, the forensic investigation indicated that CHPW member data, including member names, provider names, and health insurance claim information, may have been accessed without authorization on March 12, 2016. The investigation did not reveal any evidence that anyone misused or attempted to misuse member data.

The approximately 1,400 affected members will be sent letters from CHPW and Summit with information about how to receive one year of free credit monitoring and identity restoration services.

If you're concerned about your privacy or data, please contact Customer Service at: 1-800-440-1561 (TTY Relay: Dial 711)
